Keep the reason beside the choice.
This fictional project log records three choices for a café website. It preserves the reasoning available at the time, including conditions that would justify reopening a decision.

D-01: a smaller first release
The team chose a five-page site to answer visitor questions first. An ordering integration would add operational work that the café is not prepared to own in this example.
D-02: content people can read
Readable page text keeps menu information available without requiring a download. The owner must still maintain the source whenever dishes or prices change.
D-03: coherent feedback
One person consolidates comments so the studio receives a clear response. This does not prevent others from contributing; it identifies who resolves conflicting requests.
